mySQL Connection Details

I am trying to install a GUI for my mySQL database hosted on HM.

It asks for the following details - what should I be using ?.

Server :
Host :
Port :

Server is generally just your db name (domain_com). Host will always be 'localhost'. Port can usually be left blank, to use the default port.
